Diamond Membership offers six months of nutritional injectable and intravenous vitamins and supplements worth $2,100 for only $1,500 with a one-time payment. Tax will be included upon checkout. • 30 INJECTIONS OF CHOICE, EXCEPT NAD and INJECTABLE WEIGHT LOSS, WORTH $750- $1,500 • 6 SESSIONS OF Build Your Own Cocktails with 2 ADD-ONS worth $1,500 OR 2 sessions of NAD IV therapy worth $ 1,600, cannot be both. • $50 off on your first billing cycle. • Shareable up to 3 family members. Policy: Non-Refundable Enrollment in the Nutritional IV Therapy Program is final and cannot be canceled. Transfer Option: Enrollment may be transferred under approved circumstances. Requests must be made in writing and approved by Sei Favolosa, LLC. The new participant must meet program eligibility criteria. A transfer fee may apply. Transfer Procedure: Original participants must submit a written transfer request to Sei Favolosa, LLC, including details of the new participant. Sei Favolosa, LLC will review and notify both parties of the decision. If approved, the new participant completes necessary paperwork and meets with a healthcare provider. Continuation: Transferred participants continue remaining IV therapy and injections from the original participant's point.
